Occasionally City staff may need to conduct surveys that collect personal data. Before collecting City personal data, you must ensure that you use an approved survey platform.
Please note that the following survey platforms are approved for use at City (all other platforms should be avoided):
- Form Assembly; and
If you wish to use an alternative platform, please consult with the Information Compliance team first as we will need to undertake necessary due diligence before it can be approved.
Finally, please also note that if you choose to collect personal data in a survey, you should also include a bespoke privacy notice on the survey itself. Guidance on how to prepare a privacy notice is available here (login required).
